Please use the form below to provide the necessary ID to process a DBS (Disclosure and Barring Service) check.

If you have been told that you need to complete a DBS, then before you start, please read the Standard and Enhanced DBS check Privacy Policy (opens in new tab).

If you already have a DBS then you do not need to do it again.

All the information you need will be in the email you received.

Please provide a scan of each document. You may also provide a photograph, but please ensure:

  • No edges are cut off
  • The information is clearly visible

The information you provide will only be used to carry out a DBS check.

What is a DBS check?

The Disclosure and Barring Service helps employers make safer recruitment decisions each year by processing and issuing DBS checks for England, Wales, the Channel Islands and the Isle of Man. DBS also maintains the adults' and children's Barred Lists and makes considered decisions as to whether an individual should be included on one or both of these lists and barred from engaging in regulated activity.

There are different combinations of ID documents that are acceptable.

You must provide one document from group 1, and two more documents from groups 1 or 2a or 2b. One of the documents must verify your current address.

Note - If a document in the list of valid identity documents is:

  • Denoted with * - it should be less than three months old
  • Denoted with ** - it should be issued within the past 12 months
  • Not denoted – it can be more than 12 months old

• Letter of sponsorship from future employment provider (non UK, non EEA only) - valid only for applicants residing outside UK at time of application.)
• Utility Bill (UK)* – NOT Mobile Telephone
• Benefit Statement* - e.g. Child Allowance, Pension
• A document from Central/Local Government/Government Agency/ Local Authority giving entitlement (UK & Channel Islands)*- e.g. from the Department for Work and Pensions, the Employment Service, Customs & Revenue, Job Centre, Job Centre Plus, Social Security
• EU National ID Card
• Cards carrying the PASS accreditation logo (UK)
• Letter from Head Teacher or College Principal (16/17 year old in full time education. Only to be used in exceptional circumstances when no other documents available.)

Examples of acceptable combinations

  • Current Valid Passport [group 1] ✔️
  • Current Driving Licence (Photocard) [group 1] ✔️
  • Birth Certificate (Original) [group 1] ✔️
  • Current Valid Passport [group 1] ✔️
  • Current UK Driving licence (old style paper version) [group 2a] ✔️
  • Bank Statement (less than 3 months old) [group 2b] ✔️

Examples of unacceptable combinations

  • Certified copy of Birth Certificate [group 2a] ⚠️
  • P45 [group 2b] ⚠️
  • Mortgage Statement [group 2b] ⚠️

Although these are all acceptable documents, in order to process the DBS check you would need to provide at least one document listed in group 1

  • Expired passport ❌
  • Council Tax statement issued more than 12 months ago ❌
  • Mobile phone bill ❌

Please ensure ID documents are current. Where they are denoted with *, please ensure documents are less than three months old. Where documents are denoted with **, please ensure that they are less than 12 months old. Please note: Mobile Phone bills are not acceptable.
